To process your refund, transfer, or misapplied payment, please submit proof of payment (canceled check, bank statement, or online statement) and provide the refund mailing address if different from the service address.

To proceed with your refund, we need proof of payment. Please submit one of the following documents:
- A canceled check (front and back)
- A bank statement
- An online statement that clearly shows your name and the relevant transactions
Additionally, if the refund mailing address or payee differs from the service address or owner, please provide that information as well.
